APPOINTMENTS
Patients are seen by appointment only. To schedule an appointment call 210-404-0000. Please call in advance for routine office visits and schedule follow-up appointments as you leave. We make every effort to stay on schedule, however emergencies do arise. If we are seriously delayed, we will attempt to notify patients beforehand or upon arrival. As a courtesy to other patients and staff please call the office as soon as possible if you are unable to keep your appointment or are going to be late.

TELEPHONE CALLS
Telephones are answered Monday through Thursday 8:30am to 5:00pm and Friday 8:30am to 12 noon.
Please call the office if you have any questions regarding your condition, medications, or treatments. Our call specialists are trained to take a message and forward it to your nurse and doctor. Your doctor's nurse will return your call and assist you. Urgent calls will be handled immediately.

EMERGENCY CALLS
One of our physicians can be reached 24 hours a day by calling 210-404-0000. After hours calls should be for emergencies and urgent matters only. In a life threatening emergency, call 911.

PRESCRIPTIONS
All prescriptions and refill requests should be addressed with your physician at the time of your visit. Should you require a refill outside of a visit, please contact your pharmacy 2 to 3 days prior to your prescription expiring and have them fax a request to our office at 210-404-2812 or 210-404-2813. All requests will be processed within 48 hours. We are unable to call refills to mail order pharmacies but we will be happy to mail your prescription(s) to you or you may obtain your written prescription(s) at our office.

LABORATORY TESTING
As a service to our patients we have a laboratory in our office. It is open Monday through Thursday 8:00am to 5:00pm and Friday 8:00am to 12:00noon.

The hospital of choice for our practice is Methodist Stone Oak Hospital.

INSURANCE AND YOUR RESPONSIBILITY
Prior to your appointment, please check your insurance information so you will be informed about referrals, co-payments, and any deductible required. We accept: cash, checks, Visa, MasterCard, Discover and American Express.
For your first visit, please bring your insurance card and arrive early to complete the necessary patient information forms.
We accept Medicare as well as most insurers, however, please review all insurance information with our staff prior to services being rendered.
Please notify us in advance if your insurance requires you to use a specific contracted lab or x-ray facility.
Your health insurance contract is between you and your insurance company. Any complaints regarding your coverage should be directed your carrier.

REFERRALS
Please allow 72 hours for referral processing. If you are being referred to a specialist, your nurse will submit a referral request to our referral coordinator. |
